PRIVACY NOTICE

Why I collect your personal data and what I do with it.

When you supply me with your personal details they are store and processed for 4 reasons:

  1. I need to collect personal information about your health in order to provide you with the best possible treatment.  Personal information consists of Name, Address, Date of Birth, telephone numbers and email address. Personal information regarding your reason for seeking acupuncture is recorded, including current medical health, past medical history, family history, current medications and general health questions. Your requesting treatment and my agreement to provide that care constitutes a contract.  You can, of course, refuse to provide the information, but if you do that I would not be able to provide treatment.
  2. I have a “Legitimate Interest” in collecting your information, because without it I couldn’t do my job effectively and safely.
  3. It is important that I can contact you in order to confirm your appointments or to update you on matters related to your medical care.  This again constitutes “Legitimate Interest”, but this time it is your legitimate interest.
  4. Provided I have your consent, I may occasionally send you general health information in the form of articles, advice, or newsletter.  You may withdraw this consent at any time by emailing me via this website, or by contacting me directly via text or telephone (details below).  Consent will be obtained from you during your initial consultation using an electronic patient consent form.

I have a legal obligation to retain your records for 7 years after your most recent appointment (or age 25, if this is longer), but after this period I will delete your records.

Patients records from April 2018 are stored electronically by a specialist medical software company called Write-Upp. The information is not shared by any other person, organisation, or third party companies.  All information held in computer, tablet, and mobile phone is password protected and have encryption techniques to keep information secure.  The devices have built-in security controls, and are not shared by any other member of the household.  Paper notes held prior to April 2018 are stored in a locked filing cabinet, at 22 Holmhead Drive, Stewarton KA3 5QR.  I am the only person with access to this filing cabinet.

I will never share your data with anyone who does not need access without your written consent.

You have the right to see what personal data I hold of yours, and you can also ask me to correct any factual errors.  Provided the legal minimum period has elapsed, you can also ask me to erase your records.

I want you to be absolutely confident that I am treating your personal data responsibly, and that I am doing everything I can to make sure that the only people who access that data have a genuine need to do so.  If you feel that I am mishandling your personal data in any way, you have the right to complain.  Complaints need to be sent to the “Data Controller”, which is myself, Alexandra Gilmartin, Telephone: 07967134246, Email: [email protected]

If you are not satisfied with my response, then you have the right to raise the matter with the Information Commissioner’s Office.
